Job Summary

CenterWell Senior Primary Care is seeking a highly skilled and compassionate Nurse Practitioner to join our team in Lakewood and surrounding areas. As a key member of our primary care team, you will provide high-quality, patient-centered care to seniors in their homes, focusing on proactive, preventive care and chronic condition management.

Responsibilities
  • Provide comprehensive primary care services to seniors in their homes, including wellness visits, physical exams, and chronic condition management.
  • Work collaboratively with our interdisciplinary care teams to develop and implement personalized care plans that address the unique needs and goals of each patient.
  • Conduct thorough assessments, diagnose and treat acute and chronic conditions, and prescribe medications as needed.
  • Stay up-to-date with the latest clinical guidelines and best practices in primary care, and apply this knowledge to improve patient outcomes and quality of care.
  • Communicate effectively with patients, families, and caregivers to ensure seamless care coordination and patient satisfaction.
  • Participate in quality improvement initiatives and contribute to the development of policies and procedures that promote high-quality, patient-centered care.
Requirements
  • Master's degree in Nursing (MSN) or Doctor of Nursing Practice (DNP) degree.
  • Active, unrestricted state license to practice as a Nurse Practitioner.
  • Board certification in a relevant specialty (e.g., Family Nurse Practitioner, Gerontological Nurse Practitioner).
  • Minimum 2 years of experience as a Nurse Practitioner in a primary care setting.
  • Strong clinical skills and experience in chronic condition management, including diabetes, hypertension, and heart failure.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
  • Ability to work independently and as part of a team, with a strong commitment to patient-centered care and quality improvement.
Preferred Qualifications
  • Experience working in a home-based primary care setting or with a similar population.
  • Knowledge of value-based care and HEDIS quality metrics.
  • Experience with electronic medical records (EMRs) and other healthcare technology.
