Job Overview

We are seeking a highly skilled Registered Nurse Case Manager to join our team at Joseph's House, a community-focused organization dedicated to providing comprehensive care services. The ideal candidate will possess excellent clinical skills, effective communication abilities, and a passion for working with vulnerable populations.

Responsibilities
  • Triage and Admission Process: The successful candidate will be responsible for triaging new residents, ensuring seamless admission processes, and coordinating care transitions.
  • Resident Care Oversight: This role involves overseeing resident care, including symptom management, medication administration, and daily personal care.
  • Care Protocols Development: The RN Case Manager will develop and maintain resident care protocols, ensuring high-quality care delivery.
  • Case Management: This role encompasses case management of residents requiring end-of-life care or medical stabilization and transition back to the community.
  • Interdisciplinary Collaboration: The RN Case Manager will work closely with healthcare providers, staff, and community partners to ensure comprehensive care delivery.
  • Training and Supervision: This role involves training and supervising resident care staff and volunteers.
  • On-Call Responsibilities: The successful candidate may be required to accompany residents to medical appointments and participate in on-call rotations.
Qualifications
  • Registered Nurse License: A current, active license to practice as a Registered Nurse (RN) in the District of Columbia is mandatory.
  • Experience: Five plus years of experience as a Registered Nurse, preferably in a similar setting.
  • Vulnerable Population Experience: Past experience working with vulnerable populations and a genuine desire to serve this community.
  • Collaboration Skills: Ability to collaborate effectively in a multicultural environment.
Requirements
  • Valid Driver's License: A valid driver's license and ability to drive a vehicle.
  • Proof of Vaccination: Proof of COVID-19 vaccination.
Salary and Benefits
