Job Overview
We are seeking an experienced Chronic Care Program Administrator to join our team at Unity Health Care. As a key member of our Chronic Care team, you will be responsible for overseeing the non-clinical aspects of our chronic care programs.

About the Role
The Chronic Care Program Administrator will collaborate with Discipline Directors and Chronic Care leadership to assess, formulate, implement, manage and monitor Unity's patient-centered chronic care programs and services. This role requires strong program management skills, excellent customer service skills, and the ability to maintain confidentiality and professionalism in all interactions.

Main Responsibilities
* Responsible for oversight and management of non-clinical aspects of Chronic Care Programs
* Collaborates with appropriate Discipline Directors and Chronic Care leadership to assess, formulate, implement, manage and monitor Unity's patient-centered chronic care programs and services
* Responsible for compliance monitoring for all federal, state, and local requirements related to Unity's chronic care programs
* Leads team(s) in annual goals setting that align with Unity's organizational goals and national recommendations
* Prepares proposals, progress reports, and facilitates program evaluation activities
* Develops and monitors budgets; ensures appropriate expenditure allocation

Requirements
To be successful in this role, you will need:
* A Master's Degree in Public Health or Health Care Administration, or equivalent combination of education and experience
* Two years of professional program management experience in a community-based health program
* Excellent communication and interpersonal skills
* Ability to analyze and systematically compile technical and statistical information
* Strong organizational skills and attention to detail

Benefits
Unity Health Care offers a competitive salary range of $70,000 - $90,000 per year, depending on experience. Our comprehensive benefits package includes medical, dental, and vision insurance, 401(k) matching, and generous paid time off. We also offer opportunities for professional growth and development in a dynamic and supportive work environment.
Estimated Salary: $80,000 per year.
