About the Role

We are seeking a highly skilled RN Care Coordinator Inpatient to join our team at PeaceHealth. As a key member of our care management team, you will play a critical role in coordinating and overseeing the plan of care for selected inpatient populations across the continuum of illness.

Key Responsibilities
  • Review patient admission data to determine the suitability of the level of care in accordance with hospital-sanctioned ISDA criteria and/or other established criteria for other settings across the continuum of care.
  • Assess, develop, implement, and monitor a comprehensive plan of care through an interdisciplinary team process in conjunction with the patient and family in internal and external settings.
  • Comprehensively assess patients' goals, biophysical, psychosocial, environmental, and discharge planning needs, as well as their financial status.
  • Identify key problems, strengths, and resources to be addressed in the plan of care, which may include over or underutilization of services, premature discharge from an appropriate level of care, need for specific services to improve health status, caregiver fatigue and burden, knowledge deficits, and inability to comply with the plan of treatment.
  • Communicate utilization assessments needed to justify continued hospitalization and to obtain procedures for pre-certification.
Requirements
  • Accredited School of Nursing Required: Nursing.
  • Minimum of 3 years Required: Clinical nursing experience in an acute care hospital.
  • Preferred: Utilization management/quality management experience.
  • Required: Registered Nurse in state of practice.
  • Required: Basic Life Support.
  • Preferred within 3 years: Specialty Certification.
Working Conditions
  • Lifting: Patient handling no greater than 35 lbs. without the use of assistive equipment and/or devices (NIOSH).
  • Fine motor skills to be able to grasp and control medical equipment and perform precise procedures.
  • Push/Pull: Frequently up to 45 lbs. force (i.e., WOW, medical carts).
  • Perform hands on CPR at least 20 mins using lbs. of force (National Assoc. of EMS Physicians and AHA).
  • Ability to move around area with frequent sitting.
  • Bending/ stooping/ squatting/ reaching/ kneeling frequently.
  • Lifting (non-patient) up to 20 lbs. occasionally and 5 lbs. frequently.
Environmental Conditions
  • Exposure to biohazard, body fluids, and airborne particles.
  • Must be able to complete tasks in a noisy environment.
  • Vision and hearing required within normal limits (glasses, contacts, hearing aids permitted).
  • Ability to communicate and exchange accurate information.
