Position Overview:
PeaceHealth is seeking a Registered Nurse Care Coordinator for Inpatient Services.

Job Summary:
This role involves comprehensive care management for a Full Time, 1.00 FTE, Day position. The compensation for this position is competitive, with a salary range of $65.72.

The hiring rate is influenced by various factors such as education, training, work experience, and other relevant criteria.

Potential hiring bonuses and relocation assistance may be available.


Key Responsibilities:
  • Coordinate and oversee care plans for selected inpatient populations throughout their healthcare journey.
  • Promote effective utilization and monitoring of healthcare resources.
  • Lead interdisciplinary teams to achieve optimal clinical, financial, and satisfaction outcomes.

Essential Functions:
  • Review patient admission data to assess the appropriateness of care levels based on established criteria.
  • Develop, implement, and monitor comprehensive care plans through collaboration with patients, families, and interdisciplinary teams.
  • Evaluate patients' goals and needs, including biophysical, psychosocial, environmental, and financial aspects.
  • Identify key issues and resources to address in care plans, including service utilization and discharge planning.
  • Communicate necessary assessments to justify continued hospitalization and facilitate pre-certification procedures.
  • Perform additional duties as assigned.

Qualifications:
Education:
Accredited School of Nursing Required: Nursing.

Experience:
Minimum of 3 years of clinical nursing experience in an acute care setting is required. Experience in utilization management or quality management is preferred.

Credentials:
Registered Nurse in the state of practice is required. Basic Life Support certification is required; specialty certification is preferred within three years.

Skills:
  • Basic knowledge of statistics.
  • Strong interpersonal skills for effective communication and assessment.
  • Proficiency in applying technical principles and concepts.
  • Ability to foster positive relationships in a patient-centered environment.
  • Basic computer skills, including proficiency in MS Office applications.
  • Effective prioritization and organizational skills.

Working Conditions:
  • Ability to lift and handle patients up to 35 lbs without assistive devices.
  • Frequent physical activity, including bending, reaching, and moving.
  • Exposure to biohazards and other environmental conditions.
