As a key member of our care team, you will play a vital role in coordinating and overseeing the plan of care for selected inpatient populations, promoting effective utilization and monitoring of healthcare resources, and assuming a leadership role with the interdisciplinary team to achieve optimal clinical, financial, and satisfaction of outcomes.
Responsibilities- Review patient admission data within designated time frames to determine suitability of level of care in accordance with hospital-sanctioned ISDA criteria and/or other established criteria for other settings across the continuum of care.
- Assess, develop, implement, and monitor a comprehensive plan of care through an interdisciplinary team process in conjunction with the patient and family in internal and external settings.
- Comprehensively assess patients' goals, biophysical, psychosocial, environmental, and discharge planning needs, and financial status.
- Identify key problems, strengths, and resources to be addressed in the plan of care, including over or underutilization of services, premature discharge from an appropriate level of care, need for specific service to improve health status, caregiver fatigue and burden, knowledge deficits, and inability to comply with the plan of treatment.
- Communicate utilization assessments needed to justify continued hospitalization and to obtain procedure for pre-certification.
- Perform other duties as assigned.
- Accredited School of Nursing Required: Nursing.
- Experience: Minimum of 3 years Required: Clinical nursing experience in an acute care hospital.
- Credentials: Required: Registered Nurse in state of practice, Basic Life Support.
- Skills: Basic knowledge of statistics, Interpersonal skills to obtain and interpret information appropriate to patients' needs and age, Skill and proficiency in applying highly technical principles, concepts, and techniques, Demonstrated ability to promote positive interpersonal relationships and patient/family centered work environment, Basic computer skills including MS Office applications, Effective prioritization skills.
Patient handling no greater than 35 lbs. without the use of assistive equipment and/or devices (NIOSH). Fine motor skills to be able to grasp and control medical equipment and perform precise procedures. Ability to move around area with frequent sitting. Bending/ stooping/ squatting/ reaching/ kneeling frequently. Lifting (non-patient) up to 20 lbs. occasionally and 5 lbs. frequently. Exposure to biohazard, body fluids, and airborne particles. Must be able to complete tasks in a noisy environment. Vision and hearing required within normal limits (glasses, contacts, hearing aids permitted). Ability to communicate and exchange accurate information.
