*This role includes an on-call rotation, which entails 1 night per week and every 3-5 weekends.*

Key Responsibilities:
  • Deliver direct oversight and daily assistance to clients primarily within their home settings.
  • Establish and nurture a professional and empathetic rapport with all clients, ensuring reliable care while respecting their boundaries and understanding their needs.
  • Organize and engage in activities with clients as specified in their care plans.
  • Participate actively in weekly team meetings and group supervision, fulfilling annual in-service training as mandated by state regulations.
  • Maintain a courteous and professional relationship with clients' families and other key supporters.
  • Comply with Stay Graceful's Standard Operating Procedures and South Dakota State Regulations pertaining to in-home care.
  • Ensure all clients, colleagues, and family members receive equal treatment and respect.
  • Accountable for the quality of services and completion of essential in-home care tasks and related training, including:

*Medication Management
*CPR Certification
*First Aid Certification
*Training in life skills and care techniques, ensuring adherence to these methods
*Daily support may involve assisting clients with household chores, laundry, meal preparation, grocery shopping, communication with family and friends, and personal grooming.
*Complete and/or ensure the completion of necessary documentation and reports by the end of each workday.

Qualifications:
  • Possess knowledge of and align with Stay Graceful's Mission Statement, Core Values, and Vision Statement.
  • Each In-Home Care Provider is expected to uphold Stay Graceful's Code of Conduct, Professionalism, and Ethics.

Additional duties may be assigned as necessary.
Education and Experience Requirements:
No formal degree is required; however, relevant experience in Human Services or a related field is strongly preferred. Candidates with strong communication skills via email, word processing, and medical record management are favored. All applicants must possess (or be eligible to obtain) a valid South Dakota driver's license, proof of insurance, and clearance through the Division of Criminal Investigation.

Physical Demands:

Ability to perform the outlined duties, including lifting 10-20 lbs., standing and walking throughout the shift, and engaging in verbal communication in person or via phone.

PLEASE NOTE: Applicants with a DUI charge within the last 5 years or those with a history of violent crimes or felonies are not eligible for employment with Stay Graceful.
