About the Role

Copa Health is seeking a highly skilled Behavioral Health Case Manager to join our team. As a key member of our integrated care team, you will play a critical role in ensuring that our service participants receive comprehensive and coordinated care.

Key Responsibilities
  • Collaborate with the integrated care team to engage, educate, communicate, and coordinate care with service participants, their family, behavioral health, medical, and dental providers, community resources, and others.
  • Develop and maintain and manage the Integrated Service Plan (ISP) within required time-frames.
  • Participate in the development of the integrated comprehensive assessment and Integrated Service Plan (ISP).
  • Implement services to members in accordance with their plan and provide ongoing care and reviews progress.
  • Coordinate with all involved healthcare, behavioral health, and wellness providers and maintain current documentation on members' progress in managing chronic medical and psychiatric conditions and assists the individual in achieving their recovery goals.
Requirements
  • High School degree required, and minimum four (4) years of experience in behavioral health and SMI, or
  • Bachelor's degree in a related field with no experience.
  • AZ valid Driver's License, vehicle with state minimum insurance coverage, and able to meet Copa Health company driving requirements and safety-sensitive requirements.
