Care Coordinator for Community-Based Care

Job Summary
">We are seeking a Care Coordinator to join our team at Computer Aid in the state of Delaware. As a key member of our community-based care team, you will be responsible for providing high-quality care coordination and management services to our members.
">About the Role
">This is a full-time 3-month contract-to-hire position that requires flexible working hours to meet the needs of our members. The successful candidate will have excellent communication skills, both written and verbal, and be able to work effectively with healthcare providers, family members, and other stakeholders to develop and implement individualized care plans.
">Key Responsibilities

- Travel to Members' Homes: Travel to members' homes, nursing facilities, and other community-based settings to conduct face-to-face needs assessments and provide support and guidance as needed.">
- Care Planning: Develop and implement individualized care plans that address the unique needs and goals of each member.">
- Coordination of Care: Coordinate care across the continuum of services, including healthcare providers, social services, and community resources, to ensure seamless transitions and optimal outcomes.">
- Education and Support: Educate members and their families about health conditions, treatment options, and available resources, and provide emotional support and guidance as needed.">
Requirements

- Current Registered Nurse License: Must hold a current RN license in the state of Delaware.">
- Case Management Experience: Minimum 5 years of experience in case management, including experience working with complex populations such as geriatric special needs, behavioral health, and home health.">
- Excellent Communication Skills: Strong communication and interpersonal skills, with ability to work effectively with diverse populations and stakeholders.">
- Computer Literacy: Proficiency in PC-based word processing and database documentation software, including Microsoft Office and electronic health record systems.">
- Reliable Transportation: Reliable transportation for travel within assigned territory, including occasional evening or weekend shifts.">
- Ability to Meet Regulatory Deadlines: Ability to meet regulatory deadlines and maintain accurate records in accordance with federal and state regulations.">
Preferred Qualifications

- Certified Case Manager (CCM): Certification as a Case Manager (CCM) or similar professional certification.">
- Licensed Bachelor's Social Worker (LBSW): Licensure as a Bachelor's Social Worker (LBSW) or similar professional licensure.">
- Medicare and Medicaid Experience: Experience working with Medicare and Medicaid populations and navigating related regulations and policies.">
Salary Range
">The estimated salary range for this role is $60,000 - $80,000 per year, depending on experience and qualifications.
