KEY RESPONSIBILITIES:
- Perform comprehensive assessments and develop tailored care plans to facilitate better adaptation to dialysis treatment.
- Support home management efforts in collaboration with the Interdisciplinary Team (IDT).
- Maintain effective communication with the IDT regarding patient concerns, interventions, and objectives.
- Consult with healthcare providers, including physicians and therapists, to coordinate patient care.
- Assist with the admissions process to address any insurance-related matters.
- Engage actively in initiatives aimed at enhancing clinical outcomes as part of the quality assurance and performance improvement (QAPI) process.
- Utilize Crown web for accurate data submission.
- Participate in individual and group projects as assigned by management.
- Perform additional duties as required.
- Uphold the values and customer service standards of the organization.
- Deliver exceptional customer service to all staff, clients, and vendors.
- Foster and maintain effective relationships through timely and clear communication.
- Proactively address and resolve customer service issues in a timely manner.
- Proficient in computer applications, particularly the Microsoft Office Suite: Outlook, Word, and Excel.
- Strong presentation and communication abilities.
- Demonstrated productivity and resourcefulness with a proven ability to meet deadlines.
- Self-motivated individual capable of working independently while thriving in a collaborative, fast-paced team environment.
- Ability to manage multiple tasks, work under pressure, and meet deadlines effectively.

- High integrity and a team-oriented approach with a focus on patient service and adaptability.
- A minimum of one year of experience as a renal social worker.
- A Master’s degree in Social Work is required.
- Must possess a Licensed Social Worker (LSW) credential.
