Job Summary

We are seeking a highly skilled Clinical Social Worker LCSW to join our Behavioral Health Outpatient Services team at Ascension St. Joseph Hospital in Elgin, IL. As a key member of our team, you will provide specialized clinical knowledge and advanced clinical skills in the assessment, diagnosis, and treatment of mental, emotional, behavioral, and developmental disorders, conditions, and addictions.

Responsibilities
  • Administer and interpret tests and measures of psychosocial functioning for the purpose of diagnosing mental, emotional, behavioral, addictive, and developmental disorders and disabilities.
  • Develop and implement appropriate assessment-based treatment plans.
  • Consult with team managers, statutory and voluntary agencies, and patient relatives to ensure compliance with the patient's treatment plan.
  • Provide crisis-oriented psychotherapy, as well as short-term and long-term psychotherapy and psychotherapeutic treatment.
  • Document patient therapy in accordance with both federal and state regulations.
Requirements
  • Licensure/Certification/Registration: Social Worker specializing in Clinical Practice obtained prior to hire date or job transfer date required.
  • Approved Local Exception preferred.
  • Drivers License preferred.
  • Licensed Clinical Social Worker (LCSW) credentialed from the Illinois Department of Financial and Professional Regulation preferred.
  • Certified Case Manager credentialed from the American Case Management Association (ACMA) preferred.
  • Certified Case Manager credentialed from the Commission for Case Manager Certification (CCMC) preferred.
  • Education: Master's degree in Social Work required.
  • Work Experience: 3 years of experience preferred. Previous experience in an acute setting preferred.
