Job Description

AURORA HOME CARE, INC. is seeking a skilled Licensed Practical Nurse to provide high-quality patient care and support. As a key member of our team, you will be responsible for creating and managing personalized care plans for individual patients.

Responsibilities

  • Collaborate with healthcare professionals to assess, plan, implement, and evaluate patient care plans
  • Administer medications and monitor patients for side effects or reactions
  • Document any changes in patient conditions and report to supervisors
  • Maintain accurate records of patient care and discuss observations with supervisors

Qualifications

  • Currently licensed Licensed Practical Nurse (LPN) in state of employment
  • Previous experience as a Licensed Practical Nurse is preferred
  • Excellent interpersonal and client care skills
  • Valid driver's license
  • First aid/CPR certification required
