POSITION: FAMILY STABILIZATION ADVOCATE - CASE MANAGER

We are looking for a Family Stabilization Advocate - Case Manager to deliver essential Stabilization Services to families transitioning from shelters or those referred by the Front Door program. This role entails providing comprehensive case management support for a duration of three years, focusing on ongoing barrier reduction, liaising with property owners to ensure adherence to agreements, and facilitating appropriate referrals to help families maintain housing stability post-Stabilization.

The Family Stabilization Advocate will employ a trauma-informed methodology to offer case management services, which includes the development of monthly Rehousing Plans, community resource referrals, advocacy, assessments, and crisis intervention, all aimed at securing permanent housing for families.

Key Responsibilities:

  • Conduct intake and needs assessments for each family, covering various areas such as housing, financial management, employment, education, nutrition, legal matters, health, parenting, and daily living skills.
  • Interview and assess clients to create Individualized Service Plans that align with program requirements.
  • Assist families in obtaining permanent housing by utilizing all available housing options, including private rentals and long-term subsidies.
  • Collaborate closely with families to identify strengths and develop strategies to overcome barriers and concerns identified during assessments.
  • Ensure compliance with EOHLC Shelter Scope of Services.
  • Support clients in creating self-sufficiency plans.
  • Guide clients towards self-advocacy roles.
  • Conduct home visits and provide transportation for clients.
  • Manage emergency response situations.
  • Set and monitor goals collaboratively.
  • Work in partnership with an interdisciplinary team of service providers; maintain accurate case documentation.

Qualifications:

  • High School Diploma or Equivalent required.
  • Prior relevant experience is essential.
  • A minimum of one year in assessment, advocacy, and case management within a Human Services context is required.
  • Experience working with low-income, culturally diverse, and at-risk families and individuals is necessary.
  • Demonstrated knowledge of community resources available to low-income individuals and families.
  • Exceptional customer service, organizational, and communication skills, both oral and written.
  • Ability to manage multiple priorities in a fast-paced environment.
  • Proficient in basic computer software and data collection systems.
  • Strong crisis management capabilities.
  • Ability to maintain accurate records and confidentiality.
  • Attention to detail is crucial.
  • Valid Driver's License required.
  • Bilingual abilities (English/Spanish, English/Haitian-Creole, English/Khmer, English/Portuguese) are preferred.
