Job Title: Case Manager

Community Teamwork Inc is seeking a skilled Case Manager to join our team in the Greater Lowell and Merrimack Valley Region. As a Case Manager, you will play a vital role in helping us end homelessness in our community.

About the Role

We are looking for a highly motivated and organized individual to work with low-income individuals and families, providing them with the support and resources they need to achieve stability and self-sufficiency. The ideal candidate will have prior experience working with vulnerable populations and a strong understanding of community resources.

Responsibilities
  • Conduct intake and needs assessments with clients, identifying areas of strength and weakness and developing individualized service plans.
  • Work closely with clients to build self-sufficiency plans, assist them in transitioning to self-advocacy roles, and advocate on their behalf as needed.
  • Establish and evaluate goals for clients, working collaboratively with interdisciplinary teams to ensure coordination of services.
  • Maintain accurate case records, documenting all services rendered and entering required data into agency databases.
  • Provide emergency response management and accompany clients to appointments as necessary.
Requirements
  • Prior experience working with low-income, culturally diverse, and at-risk populations.
  • High School Diploma or Equivalent required.
  • Excellent customer service, organizational, and communication skills.
  • Ability to manage multiple priorities and work in a fast-paced environment.
  • Proficient computer skills and knowledge of community resources.
