Home Health Speech Therapist

We are seeking a skilled Home Health Speech Therapist to join our interdisciplinary team at CenterWell Home Health. As a Home Health Speech Therapist, you will play a vital role in providing high-quality patient care and making a meaningful difference in the lives of our patients.

Key Responsibilities:
  • Evaluate and provide speech/language pathology services to patients in the comfort of their own homes or facilities, ensuring seamless care coordination and optimal communication outcomes.
  • Collaborate with our team to develop and review comprehensive treatment plans, incorporating patient-centered care and evidence-based practices.
  • Administer and interpret diagnostic tests and therapeutic treatments, empowering patients to achieve effective communication and promoting independence.
  • Monitor patient progress, record observations, and provide timely updates to ensure a smooth transition to the next level of care.
  • Empower patients to communicate effectively through instruction and training in alternative communication systems, promoting autonomy and self-advocacy.
  • Participate in discharge planning, ensuring a seamless transition to the next level of care and promoting continuity of care.
Requirements:
  • Certification of Clinical Competence in Speech-Language Pathology or Audiology granted by ASHA.
  • At least six months of experience as a speech therapist/speech-language pathologist, with a strong foundation in patient-centered care and evidence-based practices.
  • Home Health experience is a plus, but not required; however, a strong understanding of home health care principles and practices is essential.
  • Current and unrestricted license and CPR certification, with a valid driver's license and reliable transportation.
  • Excellent organizational and communication skills, with the ability to work effectively in a fast-paced environment.
