The Nurse Practitioner specializing in Palliative Care is instrumental in the management of Curo Palliative Care patients, ensuring the formulation of customized treatment plans while demonstrating outstanding clinical expertise and ethical decision-making.
Responsibilities encompass diagnosing, treating, consulting, and follow-up under the supervision of the Palliative Medical Director, as required by State regulations.In this role, you will:
- Collaborate with the Palliative Care Medical Director and the palliative care team to provide individualized care to referred patients.
- Oversee patient care from assessment to discharge, ensuring holistic care.
disease-specific changes, functional decline, nutrition, cognition, pain management, falls, infections, and the impact of illness on the patient and family.
- Create personalized care plans in collaboration with the patient, family, and the Palliative Care Team.
- Coordinate necessary care as per the Care Plan.
- Be available for evening/weekend on-call duties, delivering competent and responsive services.
- Prescribe medications and order tests within professional scope and state regulations.
- Maintain regular communication with the Palliative Medical Director, reassess care plans, and engage the care team as necessary.
- Foster effective relationships with the care team and other professionals.
- Accurately document clinical findings in the patient's medical record.
- Adhere to Palliative Medicine Quality Measures.
Skills:
- Strong leadership, interpersonal, and communication skills.
- Ability to make independent decisions and maintain motivation.
- Knowledge of Palliative Medicine principles, Medicare/Medicaid regulations, and interdisciplinary processes.
- Compliance with continuing education requirements.
- Proficiency in point of care software.
- Master's degree in Nursing from an accredited institution.
- Minimum of two years of Nurse Practitioner experience.
- Experience in Family or Adult Medicine preferred.
- Familiarity with Part B billing.
- Ability to work independently in an Advanced Practice Role.
- Current RN license and Nurse Practitioner certification.
- Collaborative agreement with a supervising physician or Palliative Medical Director.
- Part B Provider and DEA licenses.
- ACHPN certification preferred.
- Valid driver's license and current automobile insurance.
