Conviva Care Solutions is seeking a highly skilled and compassionate Physician Assistant / Nurse Practitioner to join our team. As a key member of our healthcare team, you will be responsible for providing high-quality patient care, working closely with our physicians and other healthcare professionals to ensure the best possible outcomes for our patients.
Responsibilities:
- Take medical histories, examine and treat patients, order and interpret laboratory tests and X-rays, make diagnoses, and prescribe medications.
- Work under the direction of physicians and surgeons, exercising independent judgment and decision-making on complex issues.
- Use your skills and expertise to make a positive impact on the lives of our patients.
- Collaborate with our healthcare team to develop and implement effective treatment plans.
- Provide exceptional patient care, demonstrating a high level of skill with interpersonal relationships and communications.
Requirements:
- Bachelor's degree in a related field.
- Masters in Science - NP or PA.
- Active, unrestricted NP or PA license in Florida.
- 1 or more years of adult medicine experience as an NP or PA.
- Excellent verbal and written communication skills.
- Demonstrate a high level of skill with interpersonal relationships and communications.
Preferred Qualifications:
- Bilingual (English and Spanish) is a plus.
- Knowledge of HEDIS measures and quality indicators.
- Experience managing Medicare Advantage panel of patients with understanding of Best Practice in coordinated care environment in a value-based relationship environment.
