Position: Patient Care Coordinator
Location: North Phoenix (Initial 6-8 Weeks On-Site, Then Remote)
Compensation: $18/HR
Work Schedule: Monday to Friday, 8 AM - 5 PM (Flexibility Required)
Immediate Hiring Available
Essential Qualifications:
- Experience in Healthcare
- Prior Call Center Experience (Handling 50+ calls daily)
- Exceptional Customer Service Abilities
- High School Diploma or GED Required
- Facilitate the continuity of patient services by reaching out to designated service providers through phone or external management systems.
- Engage with patients and referral sources to collect essential information and provide updates on referral processing status or outcomes.
- Employ external staffing management software to upload necessary documents for service providers, ensuring uninterrupted patient care.
- Initiate processing of new requests via email within the referral management software by the assigned team.
- Coordinate with the Start of Care team to void authorizations as required.
- Make over 60 outbound calls each day.
Monday to Friday, 8 AM - 5 PM (Flexibility Required)
