Medical Records Advocate

The Licensed Behavioral Health Case Manager provides support for both direct referrals and data-identified referrals. This requires clinical expertise and the ability to navigate the complexities involved with special needs conditions such as substance abuse, suicidality/homicide, major depression, ADHD, eating disorders, and any severe mental illness.
You will enjoy the flexibility to work remotely and provide advocacy and support to members and family members, including caregiver support and appropriate referral to applicable resources.
Key responsibilities include:
- Exhibiting excellent customer service in engaging providers in collaborative planning
- Creating and maintaining appropriate clinical records
- Participating in clinical rounds with other members of the team and external healthcare management organizations/vendors
- Completing discharge follow-up and discharge planning/support

Intermediate level proficiency in MS Office Suite, including Word, Teams, and Excel, is required. Dedicated home office space and reliable high-speed internet are also necessary.
Medical/behavioral setting experience, discharge planning experience, and experience consulting with facility and/or hospital staff to coordinate treatment plans are highly valued.
This role is open to residents only, and the salary range is $58,300 to $114,300 annually. UnitedHealth Group complies with all minimum wage laws as applicable.
