Position Title: Community Resource Information Specialist

Department: Community Resource Center

Reports To: Resource Center Program Manager

Employment Type: Full-Time

Work Schedule: Monday - Friday, 40 hours per week or as required by the role

Compensation: $40,000 per annum

POSITION SUMMARY

The Community Resource Information Specialist serves as a vital link between the public and available resources. This role involves responding to inquiries, providing assistance, and ensuring that individuals receive timely and accurate referrals to necessary services. The specialist also plays a key role in monitoring the quality of information and assistance services, conducting intake processes, and assessing eligibility for support programs.

KEY RESPONSIBILITIES

1. **Responding to Information Requests**

a. Ensure availability to the public via phone or in-person inquiries.

b. Assess individual needs and make appropriate referrals to local service providers.

c. Evaluate the urgency of requests and facilitate connections with emergency services when necessary.

d. Conduct preliminary assessments to determine the appropriateness of case management referrals.

e. Utilize various resources to research and provide accurate information.

f. Deliver timely responses to inquiries.

2. **Documentation and Follow-Up**

a. Maintain accurate records of all public contacts using designated forms and databases.

b. Assist in reporting activities as directed by the supervisor.

c. Conduct follow-ups on all information requests in accordance with established policies.

d. Engage in ongoing communication with individuals requiring additional information and support.

3. **Screening and Enrollment**

a. Identify individuals who may benefit from screening processes and conduct screenings as needed.

b. Collect relevant information following proper disclosure protocols.

c. Ensure accurate data entry and assist with referrals and enrollment tasks as assigned.

d. Provide options counseling as required.

4. **Support for Resource Center Programs**

a. Assist with various program activities as assigned by the supervisor.

b. Help maintain eligibility for services by coordinating referrals and scheduling appointments.

c. Gather necessary financial and residency documentation to support eligibility assessments.

5. **Resource Management**

a. Conduct research on available resources.

b. Update and maintain the resource database.

c. Develop and revise resource directories and informational materials.

6. **Quality Assurance and Additional Duties**

a. Participate in quality assurance activities, including surveys and assessments.

b. Perform other tasks as assigned by the supervisor.

QUALIFICATIONS

A minimum of a Bachelor's Degree in a Human Services-related field is required.

1. Excellent written and verbal communication skills.

2. Ability to complete certification for Wisconsin Long-Term Care Functional Screen.

3. Valid driver's license and reliable transportation are essential.

4. Must carry personal auto insurance with minimum coverage of 100,000/300,000/100,000.

5. Willingness to obtain Information and Referral Services certification.

6. Successful completion of Certified Application Counselor training.

7. Experience working with mentally ill and AODA populations is advantageous.

8. Bilingual proficiency in Spanish is a plus.
