A dynamic opportunity has arisen for a licensed Registered Nurse (RN) to join Tandym Group's admissions team in Brooklyn. In this key role, the Hospice Nurse (RN) will be responsible for conducting thorough assessments to admit patients to their hospice program.

  • Schedule: Monday to Friday
  • Hours: Standard business hours
Responsibilities
  • Perform comprehensive physical and psychosocial assessments of potential hospice patients to develop individualized plans of care, taking place in both private homes and facilities.
  • Admit eligible patients into the hospice program following established standards of practice set by the National Hospice and Palliative Care Organization (NHPCO).
  • Perform other duties as needed.
Requirements
  • Minimum 2+ years of experience in med-surg, community health, oncology, or hospice settings.
  • Graduate of an accredited School of Nursing.
  • NYS Registered Nurse (RN) license.
Desired Skills and Qualifications
  • Bachelor's degree in Nursing (BSN).
  • Previous hospice experience.
  • Previous intake or admissions experience.

We estimate the salary for this role to be around $85,000 per year, commensurate with experience.
