Nurse Care Coordinator

We are seeking a dedicated and compassionate Registered Nurse to join our team at Agape Care Group. As a key member of our care team, you will play a vital role in delivering high-quality care to our patients and their families.
Key Responsibilities- Evaluate patients and develop personalized care plans to meet their unique needs
- Communicate effectively with patients, families, and the care team to ensure seamless care coordination
- Collaborate with interdisciplinary teams to provide comprehensive care and support
- Participate in quality improvement initiatives to enhance patient outcomes and satisfaction

- Graduate of an accredited school of nursing with a current state license as a registered nurse
- 2 years of nursing experience in a clinical care setting (hospice experience preferred)
- Ability to sit, stand, bend, move intermittently, and lift at least 25 lbs
