About the Role

We are seeking a highly motivated and compassionate Patient Engagement Specialist to join our team at Author Health. As an Outreach Liaison, you will play a critical role in increasing patient engagement with our services and serving as a key point of contact for patients and their caregivers.

Key Responsibilities
  • Build relationships with community members, educate them on the impact of our services, and schedule intake visits with our clinical team.
  • Meet patients in their homes, at primary care offices, or other community settings to form relationships, educate on Author Health services, and schedule intake visits.
  • Conduct door-to-door canvassing to engage hard-to-reach patients and respond to Health Information Exchange (HIE) alerts to meet with patients and their caregivers in the hospital or other care facilities.
  • Develop and implement community outreach programs and events to increase awareness and engagement rates.
  • Build partnerships with local organizations, community leaders, external providers, and brokers to expand the reach of outreach efforts.
  • Keep accurate records of all patient interactions in the engagement phase and assess patients' use of technology to assist with virtual visits.
  • Identify and remove barriers to engagement with necessary resources and supports, communicate and provide education on the value of Author Health to patients and their caregivers, and successfully meet and exceed monthly engagement targets.
Requirements
  • High school diploma/GED
  • Preferred Bilingual (Spanish-English or Haitian Creole-English)
  • Strong organizational skills and verbal/written communication skills
  • Possession of driver's license and car
  • At least 1 year of experience directly related to the duties of the position
  • Passion for, and experience working with, patients with behavioral health conditions
  • Passion for, and experience working with, seniors
  • Strong interpersonal and communication skills and the ability to work effectively with a wide range of constituencies in a diverse community
  • Knowledge of community agencies and resources
  • Working knowledge of multi-system outreach programs related to health care delivery
  • Ability to communicate medical information to health care professionals, care coordinators, and patients
  • Ability to organize resources and effectively prioritize
  • Willingness to travel up to 100 miles round trip per day
Preferred Qualifications
  • Associate's or Bachelor's degree in health/human services or other relevant field
  • Bilingual (Spanish-English)
  • Direct-to-consumer sales or patient engagement at a health plan, medical facility, or another healthcare setting
