Therapist and Patient Care Specialist

Job Description: As a Home Health Rehabilitation Therapist at Prominent Career Solutions, LLC, you will play a vital role in helping patients regain their independence and mobility. Your responsibilities will include administering manual physical exercises, instructing patients in non-manual exercises and daily-living activities, and assessing and interpreting evaluations and test results to determine treatment plans in consultation with physicians.
Responsibilities: Some of your key responsibilities will include:
- Conducting start-of-care evaluations and follow-up assessments to evaluate patient progress
- Developing and implementing individualized treatment plans to meet the unique needs of each patient
- Documenting patient care services in compliance with federal, state, and local regulations
- Collaborating with other healthcare professionals to ensure comprehensive patient care
Requirements: To be successful in this role, you will need to possess a strong caring interpersonal skill set and experience working as a physical therapist in home health settings. A graduation from an accredited Physical Therapy Program is also required. Our ideal candidate will have a passion for delivering exceptional patient care and a commitment to staying up-to-date with the latest developments in the field.
Compensation and Benefits: The estimated annual salary for this role is $83,000, depending on location and experience. In addition to a competitive salary, we offer a comprehensive benefits package, including medical, dental, and vision insurance, as well as retirement savings options and paid time off.
