Job Summary

We are seeking a skilled Registered Nurse to join our team at Conway Regional Health System as a Case Manager in the Outpatient Wound Healing Center. The successful candidate will be responsible for assessing, planning, implementing, and evaluating the care of patients with wounds, as well as providing case management services to ensure seamless transitions of care.

Key Responsibilities
  • Assess patients with wounds and develop individualized care plans to promote optimal healing and outcomes.
  • Provide education to patients and their families on wound care and management.
  • Coordinate with healthcare providers to ensure timely and effective care.
  • Participate in performance improvement activities to enhance the quality of care.
  • Act as a liaison between patients, families, and healthcare providers to ensure seamless transitions of care.
Requirements

To be successful in this role, you will need:

  • A current, active Arkansas RN license.
  • Proof of high school or equivalent education.
  • A Bachelor's degree in Nursing (preferred) or equivalent experience.
