Nurse Care Manager

LHC Group is seeking a highly skilled Registered Nurse (RN) to fill the role of Care Manager in our Home Health division. As a key member of our team, you will be responsible for providing clinical leadership and coordination of care to patients in their homes.
Responsibilities:- Supervise and coordinate clinical services to ensure high-quality patient care.
- Provide and direct provisions of nursing care to patients in their homes, as prescribed by physicians and in compliance with applicable laws and regulations.
- Coordinate determination of patient home health benefits, medical necessity, and ongoing insurance approvals.
- Ensure patient needs are continually assessed and care rendered is individualized to patient needs, appropriate and reasonable, meets home health eligibility criteria, and is in accordance with physician orders.
- Oversee and assure development, implementation, and updates to the individualized patient plan of care, as appropriate.
- Current RN licensure in the state of practice.
- Current CPR certification required.
- Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation.
