Job Summary:

The RN Case Manager will work collaboratively with residents, their families, and facility staff to coordinate care received by all disciplines throughout the course of their stay. This role will ensure accurate and timely completion of all Medicare residents' MDS assessments, utilizing current RAI manual guidelines. The Coordinator will act as a resource for the Medicare MDS Case Manager RN's, LTC nurses, and all others requiring MDS support. The MDS Coordinator will work in collaboration with the rehab director and nurse manager to provide excellence in the quality of care and customer service delivered by Marwood Nursing & Rehab.

Key Responsibilities:

• Coordinate care for assigned residents using a holistic approach, collaborating with the entire interdisciplinary team and acting as liaison to families, physicians, and home care agencies as needed. • Evaluate all aspects of care and serve as the primary team member with an all-inclusive head-to-toe knowledge of the resident's medical condition and educational needs. • Act as a resource to floor staff, including nurses, nurse aides, therapists, social workers, and dieticians, to provide education, role model, and assist in care delivery as needed. • Play an active role in the discharge process, coordinating the discharge intent notice with various disciplines. • Assess the medical needs of residents prior to discharge to ensure all educational opportunities have been addressed. • Attend resident care plan meetings, representing nursing. • Meet with the nurse manager, rehab director, social workers, and dieticians daily for a brief update to ensure a steady flow of communication amongst team members. • Perform other duties as assigned. • Adhere to McLaren Health System Standards of Conduct, facility policies and procedures, as well as federal, state, and local regulations. Qualifications:

• Graduate from an accredited school of nursing. • Currently licensed to practice as a Registered Nurse and hold a valid Michigan State License. • Satisfactory performance in staff or charge nurse roles for a minimum of 2 years. • MDS 3.0 certified (or equivalent). • Demonstrate competence in assessment skills and computer literacy. • Possess good organizational ability. • Possess good oral and written communication skills.
