Job Summary

We are seeking a highly skilled and experienced Registered Nurse to join our team as a Dialysis Nurse Manager. In this role, you will be responsible for overseeing the complete operation and performance of the delivery of dialysis treatment and care under the terms of the contract between Rendevor Dialysis and the Skilled Nursing Facility.

Key Responsibilities
  • Oversee the complete operation and performance of the delivery of dialysis treatment and care under the terms of the contract between Rendevor Dialysis and the Skilled Nursing Facility.
  • Use professional nursing knowledge, critical thinking skills, and dialysis experience to serve as a resource to staff, Physicians, and Physician's Assistants.
  • Ensure dialysis unit operates in compliance with all ESRD, HCFA, federal, AAMI, OSHA, and company policies and procedures.
  • Responsible for training and development of Skilled Nursing Facility staff while providing clinical oversight to non-licensed support team.
  • Monitor and assure patients receive adequate medical attention during dialysis treatments and pertaining to dialysis care while a patient of the Skilled Nursing Facility in the dialysis unit.
  • Conduct comprehensive assessments that include the medical, behavioral, pharmaceutical, and social needs of the patient, identify gaps in care, and barriers to good health.
  • Create and implement a care plan that will address identified needs, remove barriers to care, and improve the health of the patient.
  • Coordinate care by serving as the advocate and resource for the patient, their family, and their provider(s).
  • Facilitate continuity of care, spanning settings such as the home, hospital, skilled nursing facility, and acute care facility.
  • Manage patients during periods of transitions of care to minimize avoidable readmissions.
  • Assess the patient's knowledge of their renal condition and provide education and self-management support.
  • Provide ongoing reassessment and follow-up to improve patient outcomes.
  • Reviews and maintains updated policies and procedures on a regular basis.
  • Develops and monitors quality control measures to assure continuous quality of patient care.
  • Facilitates Quality Assurance Meetings.
  • Participates in the IDT and QAPI committee and attend meetings as needed and/or requested.
  • Identifies and reports concerns, issues, and opportunities for improvement when identified.
  • Participates in performance improvement projects within licensure, certification, and scope of practice.
  • Participates in annual QAPI training and other continuing education, as assigned, and required.
  • Provides suggestions for alternate strategies for indicators not improving.
  • Assists in building and maintaining a facility/clinic culture of safety and quality.
  • Reports errors and near misses so that steps can be taken to prevent recurrence.
  • Reports any negative feedback, complaints, grievances from other staff members, patients, and families.
  • Uses evidence-based guidelines and proper infection control practices.
  • Maintains open communication with Skilled Nursing Facility Administrator, Medical Director, and Rendevor Dialysis management and staff.
  • Utilizes resources to improve skills, knowledge, and practice to assure continuous quality of patient care.
  • Promotes a teamwork approach by offering information, advice, and assistance to all unit staff members in a positive, courteous, and cooperative manner.
  • Checks email at least weekly and responds appropriately and timely; uses company-provided email for all work-related email communications.
  • Maintains regular and predictable attendance and punctuality.
  • Adheres to all company policies and procedures.
Requirements
  • Graduate of an accredited nursing program required; Associates Degree in Nursing (ADN), Bachelor of Science in Nurse (BSN), and/or Master of Nursing (MSN).
  • Current state licensure as a Registered Nurse.
  • Current CPR certification (AHA BLS).
  • 18 months of clinical nursing experience to patients on maintenance dialysis within the last 24 months.
Work Environment

This job exists in the medical unit of a post-acute facility. This role may include exposure to chemicals and blood-borne pathogens. This role routinely involves the use of both medical and standard office equipment.

Physical Demands

The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. While performing the duties of this job, the employee is regularly required to talk or hear. The employee frequently is required to stand; walk; use hands to handle or feel; and reach with hands and arms. This position requires the ability to push, pull, or lift supplies and products up to 50 pounds. Specific visual abilities required by this job include close vision and ability to adjust focus. The employee is required to work the full schedule, which may range from five to fifteen hours in any given day, based on the unit requirements (i.e., patient census, staffing needs, etc.).
