Job Summary

We are seeking a highly skilled Renal Dietitian to join our team. As a Renal Dietitian, you will be responsible for developing and implementing nutrition plans for dialysis patients in Skilled Nursing Facilities. You will work closely with our multidisciplinary team to provide exceptional patient care and support.

Responsibilities
  • Develop and implement nutrition plans for dialysis patients
  • Collaborate with Skilled Nursing Facility dietary team to establish and update patient plans of care
  • Provide ongoing nutrition assessment and modify plans as needed
  • Work with patients and IDT to establish and document patient plans of care
  • Provide education and support to patients and families on recommended nutrition and dietary modifications
  • Maintain accurate and up-to-date patient records
  • Participate in quality improvement initiatives and contribute to a culture of safety and quality
Requirements
  • Bachelor's degree in Nutrition, Dietetics, or similar field
  • Minimum 12 months of professional work experience in clinical nutrition as a Registered Dietitian
  • State licensure, if required or available in the state where facility is located
  • Excellent verbal and written communication skills
  • Computer proficiency
Work Environment

This role will be based in a hospital or medical unit of a post-acute or correctional facility. You will be exposed to chemicals and blood-borne pathogens. You will use medical and standard office equipment regularly.

Physical Demands

This role requires regular talking and hearing. You will frequently sit, stand, walk, use hands to handle or feel, and reach with hands and arms. You may occasionally push, pull, or lift supplies and products up to 50 pounds.
