Job Summary

We are seeking a skilled Renal Dietitian to join our team. As a Renal Dietitian, you will be responsible for developing knowledge and understanding of nephrology nutrition for clinically complex dialysis patients. You will work in a challenging clinical setting and utilize your natural gift as a patient advocate to drive differential results for dialysis patients in Skilled Nursing Facilities.

Responsibilities
  • Maintain professionalism within all aspects of job functions and execution
  • Demonstrate Rendevor Dialysis mission and values daily
  • Display and maintain a positive attitude and promote same to others
  • Display ability to multi-task, prioritize, problem solve, and handle stressful situations in a timely manner
  • Know and follow policies and procedures of Rendevor Dialysis; monitor staff compliance and communicate results timely to leadership
  • Demonstrate strong communication skills, visibility, and establish productive relationships with Rendevor Dialysis team, SNF partners, vendors, and medical directors
  • Actively participate in facilities' CQI process
  • Participate in the IDT and QAPI committee and attend scheduled meetings
  • Identify and report concerns, issues, and opportunities for improvement when identified
  • Participate in performance improvement projects within licensure and scope of practice
  • Participate in annual QAPI training and other continuing education as assigned and required
  • Provide suggestions for alternate strategies for indicators not meeting goal or improving
  • Assist in building and maintaining a facility/clinic culture of safety and quality
  • Report errors and near misses so that steps can be taken to prevent recurrence
  • Report any negative feedback, complaints, grievances from other staff members, patients, and families
  • Use evidence-based guidelines and proper infection control practices
  • Display a proactive approach and be assertive in all aspects of governing unit functions
  • Utilize resources to improve skills, knowledge, and practice to assure continuous quality patient care and delivery of Rendevor Dialysis service
  • Knowledgeable in ESRD, CDC, OSHA, AAMI, NKF, Network, and Medicare standards
  • Act as the renal dietary resource to managers, units, Nephrology services, and vendors
  • Carry out MD standing orders and protocols
  • Maintain patient-specific nutritional information in EHR system
  • Develop nutrition prescription for patients in collaboration with Skilled Nursing Facility dietary team and implement orders based on Rendevor Dialysis policies and procedures and state and federal regulations
  • Provide ongoing nutrition assessment of patients based on lab results. Modify nutrition goals and dietary recommendations as needed and on a regular basis
  • Work with patient and IDT to establish, document, and update patients' plan of care
  • Instruct Skilled Nursing Facility/patient/family as appropriate on recommended nutrition/dietary modification using an individualized approach
  • Check email at least weekly and respond appropriately and timely; use company-provided email for all work-related email communications
  • Demonstrate and maintain regular and predictable attendance and punctuality
  • Adhere to company policies and procedures
  • Perform other duties as assigned
Requirements
  • Bachelor's degree in Nutrition, Dietetics, or similar area required
  • Minimum of 12 months professional work experience in clinical nutrition as a Registered Dietitian
  • State licensure, if required or available in the state where facility is located
Work Environment

This job exists in a hospital or in the medical unit of a post-acute or correctional facility. This role may include exposure to chemicals and blood-borne pathogens. This role routinely involves the use of both medical and standard office equipment.

Specific visual abilities required by this job include close vision and ability to adjust focus.
