Job Summary

Carefully coordinate oncology patient care across all continuum sites, navigating complex barriers to deliver exceptional results. Collaborate with patients, families, and caregivers to create personalized plans, focusing on compassionate and timely interventions.

Salary

$80,000 - $110,000 per annum, based on experience and qualifications.

Responsibilities

  • Evaluate new patient cancer referrals, establishing prompt contact and facilitating initial consultations.
  • Review patient records for NCCN guidelines, identifying key diagnostic and treatment stages, and scheduling necessary tests.
  • Conduct comprehensive assessments of patients' needs and barriers, ensuring timely referrals to support services.
  • Develop and implement assessment tools, care management guidelines, pathways, and algorithms for seamless cross-continuum care.
  • Promote patient education, collaboration, and self-management, enhancing overall patient experience.
  • Participate in inter-disciplinary teams to develop, implement, and evaluate new processes of care, driving continuous improvement.

Required Skills and Qualifications

  • Bachelor's Degree in Nursing.
  • 3 years of Oncology experience.
  • Licensed to practice as a Registered Nurse in South Carolina.
  • One of the following oncology certifications: OCN (Oncology Certified Nurse) from ONCC or ONN-CG (Oncology Nurse Navigator-Certified Generalist) from AONN.
