About the Role

We are seeking a highly skilled and compassionate Family Preservation Specialist - Case Manager to join our team at Samaritas. As a key member of our family preservation program, you will play a critical role in supporting at-risk families and helping them achieve stability and self-sufficiency.

Key Responsibilities
  • Provide intensive, short-term or long-term in-home crisis intervention and family education to at-risk families, based on program specifications.
  • Develop and implement individualized treatment plans, working closely with families to identify and address their unique needs and challenges.
  • Conduct regular home visits, assessments, and progress monitoring to ensure families are receiving the support they need to achieve their goals.
  • Collaborate with referral sources, agencies, institutions, and individuals who have important roles in the family's life situation, providing advocacy and professional consultation as needed.
  • Participate in team meetings, keeping all members informed of case status and progress, and providing oral and written reports as required.
  • Assist clients in clearly defining presenting problems, devising methods and goals for resolution, assessing their progress toward treatment goals, and planning termination.
  • Present cases to supervisors for review of treatment plans and progress on a regularly scheduled basis.
  • Provide appropriate referrals for clients requiring additional or alternative services.
  • Assume therapeutic responsibility for all cases on assigned caseload, conduct intake, assessment, and scheduling of all assigned clients.
Requirements
  • Minimum of a Bachelor's degree in any human service field or education preferred, or two years' experience working with children and/or families and an associate degree or the equivalent in human services or education.
  • Valid driver's license with a good driving record; reliable ongoing personal transportation suitable for safely transporting clients; current automobile registration and proof of automobile insurance.
  • Field experience working with multi-problem children and families, with the ability to relate to and engage with them, is preferred.
  • Experience in strength-based, family-centered, solution-focused approaches is beneficial.
