Overview
The Outpatient Registered Nurse (RN) CAP 3 is a key role within the Clinical Advancement Program (CAP), achieved through progression from RN CAP 2. This position is responsible for delivering and coordinating high-quality clinical care, which includes assessment, planning, intervention, and evaluation for a designated patient group. The RN may delegate appropriate tasks to direct patient care staff, including RNs, LVN/LPNs, and Patient Care Technicians. As part of the End Stage Kidney Disease (ESKD) healthcare team, this role engages in decision-making, educational initiatives, leadership responsibilities, and quality improvement efforts to enhance patient care outcomes and operational efficiency.

Key Responsibilities
All tasks and responsibilities must be executed in compliance with company policies, nursing standards, and applicable regulations.


Conducts systematic collection and analysis of patient data before, during, and after hemodialysis treatment for assigned patients, documenting findings in the medical record, making necessary adjustments to treatment plans, and notifying the Team Leader, Charge Nurse, Supervisor, or Physician as required.


Evaluates and documents the educational needs of patients and their families to provide ongoing education.

Collaborates with direct and ancillary patient care staff to ensure the safe and effective delivery of hemodialysis therapy.

Administers prescribed medications in accordance with established protocols and documents the medical rationale and outcomes.


Initiates or assists with emergency response protocols.


Acts as a resource, leader, mentor, and role model for both new and existing staff, exemplifying professional behavior and work ethics.

Ensures accurate laboratory collection, processing, and shipping procedures are followed, rescheduling any missed or inadequate laboratory collections.


Identifies expected outcomes and updates nursing assessments and care plans for assigned patients in collaboration with the Interdisciplinary Team.

Ensures patient understanding of transplant and treatment modality options.


Participates as a lead or member in educational or quality improvement initiatives as directed by the Supervisor.

May serve as a Preceptor for new hires.

Must fulfill CAP requirements to maintain or advance in the program.

Performs additional duties as assigned by the Supervisor.

Physical Requirements and Work Environment

This role involves direct patient care, which frequently requires heavy lifting, patient mobility assistance, and ambulation support. Equipment aids and/or coworkers may assist as needed.

Frequent prolonged periods of standing are required, and the employee must be able to bend over.

Occasionally, the employee may need to move machines and equipment weighing up to 200 lbs. and lift chemical and water solutions weighing up to 30 lbs. to a height of 5 feet.

The work environment is typical of a healthcare facility, featuring controlled air temperature and moderate noise levels.

Exposure to infectious and contagious diseases/materials may occur.

Daily tasks include desk work, computer usage, and interaction with patients and healthcare staff.

This position may require travel to training sites or other facilities.


May be required to perform essential functions in various locations with similar physical demands and working conditions.


Supervision
Responsible for overseeing Patient Care Technicians/LPNs/LVNs/RNs as a Team Leader or designated Nurse in Charge after fulfilling the following criteria:
3 months of experience in chronic/acute hemodialysis as a Registered Nurse.

Education and Licensure
Graduate of an accredited School of Nursing.

Current state licensure as required.

Current or successful completion of CPR/BLS Certification.

Must meet practice requirements in the state of employment.

Experience and Required Skills
Minimum of 5 years of experience in Nephrology Nursing as a Registered Nurse.

Chronic/acute Hemodialysis experience is preferred.
