Hospice Care Assistant

We are seeking a dedicated and compassionate Hospice Aide/CNA to join our team at Heart of Hospice, a part of LHC Group. As a Hospice Aide/CNA, you will play a vital role in providing personalized care and support to patients and their families.
Key Responsibilities- Provide direct patient care services as established and delegated in the Plan of Care daily.
- Assist with daily living activities, such as mobility, transfers, walking, grooming, bathing, dressing, eating, and toileting.
- Perform incidental household services essential to the patient's health care at home.
- Participate in the QAPI plan or process, identifying any problems and reporting to the administrator as needed.
- Provide emotional support and compassionate care to patients and their families.
- Successful completion of a competency evaluation prior to patient care.
- Current CPR certification.
- Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
- Mature and able to deal effectively with the demands of a hospice aide position.
- Ability to read, write, and carry out directions promptly and accurately.
