Job Summary

We are seeking a highly skilled and compassionate Social Worker to join our team on a temporary basis. As a Renal Social Worker, you will play a vital role in ensuring the well-being and dignity of our patients undergoing dialysis treatment.

Key Responsibilities
  • Provide comprehensive psychosocial assessments and develop personalized care plans for patients.
  • Collaborate with the interdisciplinary team to ensure seamless care coordination and patient outcomes.
  • Assist patients and families in navigating the dialysis process, including financial assistance, insurance coverage, and transportation.
  • Maintain accurate and confidential patient records, adhering to HIPAA regulations.
  • Participate in quality improvement initiatives and contribute to the development of facility policies and procedures.
Requirements
  • Master's degree in Social Work (MSW) from an accredited institution.
  • At least 2 years of clinical social work experience in a healthcare setting.
  • State licensure in Michigan.
