Job Title: Renal Social Worker - MSW - Perm - Detroit MI

We are seeking a highly skilled and compassionate Renal Social Worker to join our team at Della Infotech in Detroit, MI. As a Renal Social Worker, you will play a vital role in providing exceptional patient care and support to individuals undergoing dialysis treatment.

Job Summary:

The successful candidate will possess a Master's degree in Social Work (MSW) and current licensure in their state. With a minimum of one year of clinical social work experience, preferably in an ESRD setting, you will be well-equipped to handle the demands of this role. Your expertise will enable you to provide comprehensive psychosocial assessments, develop and implement care plans, and ensure seamless coordination with other healthcare professionals.

Key Responsibilities:
  • Conduct thorough psychosocial assessments to identify patient needs and develop personalized care plans.
  • Collaborate with interdisciplinary teams to ensure comprehensive patient care and support.
  • Provide education and support to patients and their families on dialysis-related topics.
  • Maintain accurate and up-to-date patient records, adhering to facility policies and ESRD regulations.
  • Participate in quality improvement initiatives and contribute to the development of policies and procedures.
Requirements:
  • Master's degree in Social Work (MSW) from an accredited institution.
  • Current licensure in your state.
  • Minimum one year of clinical social work experience, preferably in an ESRD setting.
  • Current CPR certification.
