Community Health Advocate

To be considered for this role, you must have a high school diploma with one year of relevant experience or a Bachelor's Degree. A reliable vehicle and ability to travel throughout Franklin, Hampshire, Hampden, and Northern Worcester Counties are also essential.
You should have demonstrated experience working with older adults or individuals with disabilities, including navigating benefits, resources, behavioral health systems, and long-term supports within the community. Excellent listening and oral and written communication skills, as well as computer proficiency, are also required.
This is a full-time position with a starting pay rate of $21.60/hour, plus excellent benefits, including generous PTO, medical/dental/vision insurance, flex spending, EAP, 401k, and more. A 6% pay differential for applicable bilingual skills is also available.
