Job Description

The Public Health Specialist will be responsible for coordinating community outreach efforts and providing direct support to patients in accessing needed services.

About the Role:

  • Develop and implement outreach strategies to engage patients in care and address social determinants of health.
  • Conduct screenings to assess for health-related social needs and connect patients with social service agencies.
  • Provide direct supervision, training, and support to CHW staff members.
  • Collaborate with interdisciplinary teams to develop and implement data-driven outreach strategies to improve patient health outcomes.

Requirements:

  • High school diploma or equivalent required.
  • Formal training in public health or community health or 1-3 years' related community health experience required.
  • State of Massachusetts certification as a CHW required or the ability to obtain one within one year of hire.

Estimated Salary: $60,000 - $75,000 per annum, depending on experience. We offer a competitive benefits package, including medical, dental, and vision insurance, as well as paid time off and professional development opportunities.
