Job Summary

We are seeking a highly motivated and organized Public Health Professional to serve as our Community Health Worker Team Lead. This role is responsible for overseeing the day-to-day activities of our CHW team, providing direct supervision, and ensuring the provision of high-quality patient care.

Key Responsibilities:

  • Supervise and mentor CHWs in daily problem-solving and goal attainment.
  • Provide training and support to CHWs on outreach, engagement, and population health activities.
  • Conduct outreach in community settings to raise awareness of health center services and enroll individuals in need of healthcare.
  • Collaborate with interdisciplinary teams to develop data-driven outreach strategies and improve patient health outcomes.
  • Provide additional patient support by educating clients on available health center services and programs.

Requirements:

  • High school diploma or equivalent required.
  • Formal training in public health or community health or 1-3 years' related community health experience required.
  • State of Massachusetts certification as a CHW required or the ability to obtain one within one year of hire.
  • Proficiency in written and spoken English. Bi-lingual English/Spanish, English/Moldovan, or English/Russian a plus.

Estimated Salary Range: $50,000 - $65,000 per year, depending on experience.
