Job Title: Registered Nurse Clinical Manager

At Advanced Home Health, we are seeking a highly skilled Registered Nurse Clinical Manager to join our team. As a key member of our operations team, you will play a critical role in ensuring the delivery of high-quality patient care and services.

Key Responsibilities:
  • Collaborate with scheduling department to verify employee/patient schedules and ensure accurate staffing levels.
  • Support patient needs by communicating with field staff and prioritizing patient assignments as needed.
  • Assist with order review to ensure compliance with State and Federal Regulations.
  • Maintain accurate and up-to-date Clinical Tasks Reports for variances, including Schedule Report, Missed Visit Report, and Past Due Visits.
  • Support clinical care and billing functions by educating and obtaining necessary clinical documents.
  • Verify admission criteria and communicate variances to Clinical Manager for follow-up.
  • Assist with care coordination, including DME, medical supplies, and coordination of appointments or referrals to outside agencies.
  • Facilitate patient care coordination documents with acute and post-acute facilities during patient transfers.
  • Assist with education and training of field staff.
Requirements:
  • One-year home care experience or experience in a facility setting.
  • Current, active license in good standing in the state where the agency is located.
  • Skilled in making effective verbal and written communications with subordinates, co-workers, consumers, and referral agencies.
  • Ability to educate consumers on all service lines and present to seniors and aides in a group setting.
  • High energy level and passion for care delivery.
  • Excellent organizational skills and ability to complete competing priorities.
  • Thorough understanding of home health or hospice qualifying criteria and coverage guidelines.
  • Ability to listen attentively and offer care options based on individual patient health needs.
  • Proficient computer skills, including Microsoft email, word, excel, power point, and home health or hospice agency software system.
  • Must have vehicle, current driver's license, and appropriate automobile insurance.

Work Environment: Office environment with prolonged or considerable sitting and keyboard work. Some driving/travel involved to conduct patient visits. Majority of work conducted in a fast-paced office setting. Must be able to lift 25 lbs. and handle office supplies and equipment. Considerable reaching, stooping, bending, kneeling, or crouching. Visual acuity and hearing needed to perform required job functions.
