Position Overview: Registered Nurse Case Manager

As a vital member of our healthcare team, the Registered Nurse Case Manager plays a crucial role in facilitating patient care throughout their journey, from pre-admission to discharge. Your responsibilities will include:

  • Overseeing the continuum of care for patients, ensuring smooth transitions and optimal outcomes.
  • Communicating with payers regarding patient clinical status and securing necessary approvals for extended stays.
  • Monitoring quality metrics as outlined by our Performance Improvement Plan and Medical Staff Quality Indicators.
  • Assessing patients for discharge planning needs and directing care appropriately.
  • Evaluating resource utilization to ensure effective and efficient patient care.
  • Identifying educational and psychosocial needs to enhance patient outcomes.
  • Assisting in the collection of data for quality improvement initiatives.
  • Participating in Infection Control surveillance efforts.
  • Acting as a Patient Advocate to address and resolve patient concerns.

Qualifications:

Education:
  • Graduate of an accredited nursing program; BSN preferred.
Licensure/Certification:
  • Current RN licensure is required.
  • Case Management certification is preferred but not mandatory.
  • Current BLS certification is required.
Experience:
  • A minimum of two (2) years of acute care experience is required.
  • Previous experience in case management is preferred.
Skills and Abilities:
  • Ability to work independently and manage multiple priorities.
  • Proficient in Microsoft Word and Excel.
  • Willingness to learn necessary computer applications.
  • Adaptability to changing situations and diverse personalities.
  • Proficient in utilizing Electronic Health Records for documentation.
  • Commitment to maintaining confidentiality in all patient interactions.
Key Responsibilities:
  • Collaborate with various departments to ensure high-quality patient care.
  • Work effectively with insurance companies and Medicare to ensure appropriate reimbursement.
  • Keep senior management informed of any patient care issues.
  • Coordinate with physicians to ensure accurate patient status and aftercare instructions.
  • Serve as a resource for management in patient care matters.
