Position Overview:

We are currently seeking a skilled Registered Nurse Case Manager to enhance our healthcare services.

Key Responsibilities:
  • Facilitates patient progression through the care continuum, from pre-admission to discharge.
  • Communicates with payers regarding patients' clinical conditions and secures necessary approvals for extended stays.
  • Monitors quality metrics as outlined by the Performance Improvement Plan and Medical Staff Quality Indicators.
  • Assesses patients for discharge planning needs and directs care accordingly.
  • Evaluates resource utilization for optimal efficiency; identifies educational and psychosocial needs to achieve the best patient outcomes.
  • Assists in compiling data for quality assessment and improvement.
  • Participates in Infection Control monitoring.
  • Acts as a Patient Advocate to help resolve patient concerns.

Qualifications:
  • Education: Graduate of an accredited nursing program; BSN preferred.
  • Licensure/Certification: Current RN license required; Case Management certification preferred; Current BLS certification required.
  • Experience: Minimum of two years of acute care experience required; previous case management experience preferred.
Skills and Abilities:
  • Ability to work independently and collaboratively.
  • Proficient in Microsoft Word and Excel preferred.
  • Willingness to learn necessary computer applications.
  • Adaptability to changing situations and diverse personalities with a positive attitude.
  • Proficient in utilizing Electronic Health Records for patient documentation.
  • Commitment to confidentiality in all patient and organizational interactions.
Principal Duties:
  • Collaborates with all departments to ensure optimal patient care.
  • Effectively interacts with insurance companies and Medicare to ensure appropriate reimbursement for services provided.
  • Keeps management informed of any issues affecting patient care.
  • Coordinates with physicians to ensure accurate patient status and aftercare post-discharge.
  • Serves as a resource for senior management.
