Education:
- Graduate of an accredited nursing program; BSN preferred.
- Current RN licensure is mandatory.
- Case Management certification is preferred but not required.
- Current BLS certification is required.
- A minimum of two (2) years of experience in acute care settings is required.
- Previous experience in case management is preferred.
- Ability to work independently and efficiently.
- Proficient in Microsoft Word and Excel is preferred.
- Willingness to learn necessary computer applications.
- Ability to adapt positively to changing environments and diverse personalities.
- Proficient in utilizing Electronic Health Records for documentation.
- Commitment to maintaining confidentiality in all patient interactions.
- Collaborate with various departments to ensure optimal patient care.
- Work effectively with insurance companies and Medicare to secure appropriate reimbursement for services rendered.
- Keep senior management informed of any issues related to patient care.
- Coordinate with physicians and their offices to ensure accurate patient status and aftercare instructions.
- Serve as a resource for senior management in patient care matters.
