Job Summary


Swan Hospice is seeking a skilled Registered Nurse to oversee a primary caseload of Hospice patients and provide high-quality, patient-centered care. As a key member of our interdisciplinary team, you will be responsible for assessing patient needs, developing and implementing individualized care plans, and collaborating with other healthcare professionals to ensure comprehensive care.


Key Responsibilities


  • Conduct initial and comprehensive nursing assessments to identify patient needs and develop individualized care plans.
  • Implement and evaluate the effectiveness of care plans, making revisions as needed to ensure optimal patient outcomes.
  • Collaborate with the interdisciplinary team to ensure all patient and family care needs are met.
  • Provide education and support to patients, families, and caregivers on nursing care and related issues.
  • Supervise and mentor Hospice Aides/Homemakers to ensure high-quality care delivery.

Requirements


  • Graduate of an accredited registered nursing program.
  • Minimum one year of general nursing experience, preferably in hospice, home health, medical, surgical, or critical care.
  • Experience with pain and symptom management interventions preferred.
  • Ability to work within an interdisciplinary setting and understand Hospice philosophy and principles of death and dying.
  • Registered Nurse licensed by the New Jersey State Board of Nursing.
  • Valid driver's license and reliable transportation.
