Job Summary


Swan Hospice is seeking a skilled Registered Nurse to oversee a primary caseload of Hospice patients and provide high-quality, patient-centered care. As a key member of our interdisciplinary team, you will be responsible for assessing patient needs, developing and implementing individualized care plans, and collaborating with other healthcare professionals to ensure comprehensive care.


Key Responsibilities


  • Conduct initial and comprehensive assessments of patients to identify their unique needs and develop personalized care plans.
  • Implement and evaluate the effectiveness of care plans, making adjustments as needed to ensure optimal patient outcomes.
  • Collaborate with the interdisciplinary team, including physicians, social workers, and other healthcare professionals, to ensure seamless care coordination.
  • Provide education and support to patients, families, and caregivers on disease management, symptom control, and end-of-life care.
  • Supervise and mentor Hospice Aides/Homemakers to ensure high-quality care delivery.
  • Stay up-to-date with the latest research, best practices, and regulatory requirements in hospice care.

Requirements


  • Graduate of an accredited registered nursing program.
  • Current licensure as a Registered Nurse in New Jersey.
  • Minimum one year of general nursing experience, preferably in hospice, home health, medical, surgical, or critical care.
  • Experience with pain and symptom management interventions preferred.
  • Ability to work effectively in an interdisciplinary setting and communicate with patients, families, and healthcare professionals.
  • Understanding of hospice philosophy and principles of death and dying.

Preferred Qualifications


  • Bachelor's degree in Nursing.
  • Certified Hospice and Palliative Nurse (CHPN) certification.
