We are seeking a skilled Hospice Social Worker to join our team at Res-Care, Inc. as a Patient Care Specialist. As a key member of our interdisciplinary team, you will play a vital role in providing medical social work services to patients and their families.
Responsibilities- Assess and Evaluate: Conduct thorough assessments of patients' psychosocial status, including family dynamics, caregiver abilities, and communication patterns.
- Counseling and Intervention: Provide counseling and crisis intervention as needed to patients and their families, addressing stress and coping difficulties.
- Plan and Coordinate: Develop and implement individualized plans of care, collaborating with patients, families, and healthcare professionals.
- Documentation and Compliance: Maintain accurate clinical records and ensure compliance with state and federal regulations.
- Additional Responsibilities: Participate in interdisciplinary team meetings, assist with funeral arrangements, and provide support for patients and families in planning for financial, legal, and healthcare decision responsibilities.
- Education: Master's degree in Social Work from a Council on Social Work Education-accredited program.
- Licensure: Current licensure to practice in the state of operation, according to Medicare guidelines.
- Experience: One year or more of experience in healthcare social work, with a preference for home health or hospice experience.
- Skills: Excellent verbal and written communication, organization, and clinical documentation skills.
