Job Summary

We are seeking a highly skilled and detail-oriented Care Management Specialist to join our team at Samaritan Health Services. As a key member of our social services team, you will play a vital role in supporting department staff with clerical and administrative duties, including general office work and coordination of discharge planning and utilization review processes.

Key Responsibilities
  • Perform a variety of clerical and administrative tasks to support department staff, including data entry, record-keeping, and communication with clients and healthcare professionals.
  • Coordinate discharge planning and utilization review processes, ensuring seamless transitions for patients and families.
  • Provide exceptional customer service, responding to inquiries and resolving issues in a timely and professional manner.
  • Collaborate with multidisciplinary teams, including doctors, nurses, and other medical professionals, to ensure comprehensive care and support for clients.
  • Maintain accurate and up-to-date records, adhering to confidentiality and data protection protocols.
Requirements
  • High School diploma or equivalent required.
  • Two (2) years college-level education in a healthcare-related field and/or experience in a similar role in a medical setting required.
  • Experience with computer applications and software, including word processing, spreadsheets, and database management.
  • Excellent communication and interpersonal skills, with the ability to work effectively with diverse populations and stakeholders.
  • Ability to maintain confidentiality and handle sensitive information with discretion.
