POSITION SUMMARY


We are currently seeking a dedicated LPN residing in Virginia, with an active license for the state. Candidates from the Northern Virginia area are preferred.

The Case Coordinator will operate in both remote and field environments, primarily assisting our Medicaid demographic.

Strong computer proficiency and meticulous attention to detail are essential for managing multiple systems, engaging with members via phone, and accurately documenting interactions.

This role demands a high level of productivity in a dynamic setting.

TRAVEL REQUIREMENTS


Occasional travel (up to 25%) is expected for member visits within a two-hour radius, with mileage reimbursement provided.


WORK SCHEDULE


The position follows a Monday to Friday schedule from 8 AM to 5 PM EST, with on-call duties one week per year and no obligations on holidays or weekends.


JOB RESPONSIBILITIES


Molina Healthcare Services (HCS) collaborates with members, healthcare providers, and multidisciplinary teams to evaluate, facilitate, plan, and coordinate comprehensive care delivery, including behavioral health and long-term care for high-need members.

KEY COMPETENCIES


- Conduct clinical assessments of members within regulated timelines to determine eligibility for case management based on clinical judgment and health changes.


- Develop and execute a tailored case management plan in partnership with the member, their caregivers, healthcare professionals, and support networks to meet individual needs and objectives.


- Perform ongoing evaluations of care plans to assess effectiveness, document interventions, and recommend adjustments as necessary.


- Maintain a consistent case load for regular outreach and management.


- Promote service integration for members, including behavioral health and long-term support services, to enhance continuity of care.


- Facilitate interdisciplinary team meetings and informal collaborations.


- Utilize motivational interviewing techniques and Molina clinical guidelines to educate and support members in achieving positive health changes.


- Identify barriers to care and provide necessary coordination and assistance to address member concerns.


- Collaborate with RN case managers and supervisors as required.


- Case managers specializing in Behavioral Health and Social Sciences may offer consultation and resources to peers as needed.


ADDITIONAL REQUIREMENTS


Local travel of up to 40% may be necessary, depending on the complexity of assigned cases and state-specific regulations.


EDUCATIONAL REQUIREMENTS


Completion of an accredited Licensed Vocational Nurse (LVN) or Licensed Practical Nurse (LPN) program, or a Bachelor's or Master's degree in a relevant field such as social science, psychology, gerontology, public health, or social work.


EXPERIENCE REQUIREMENTS


1-3 years of experience in case management, disease management, managed care, or medical/behavioral health settings is required.


LICENSE AND CERTIFICATION


Must possess an active, unrestricted license in good standing, along with a valid driver's license and a clean driving record.


PREFERRED QUALIFICATIONS


3-5 years of experience in relevant fields is preferred, along with certifications such as Licensed Clinical Social Worker (LCSW), Certified Case Manager (CCM), or Licensed Professional Counselor (LPC).
