Clinical Nurse Specialist

A dynamic and experienced Clinical Nurse Specialist is sought to join our team at the University of Virginia Health System. This role requires a professional clinician focused on population management with demonstrated capability to provide relationship-based care within a collaborative multidisciplinary team.
Key Responsibilities:
- Develop, implement, and evaluate an individualized plan of care that educates, empowers, and optimizes patient outcomes throughout all transitions within the continuum of care.
- Provide consultation and education to healthcare providers and the community.
- Collaborate with multidisciplinary team members to ensure comprehensive care and effective communication.
- Participate in performance improvement activities, quality assurance, and patient safety initiatives.
- Stay up-to-date with the latest clinical knowledge and best practices in transplant nursing.
Requirements:
- Bachelor of Science in Nursing from an accredited nursing program.
- 2 years of relevant experience in transplant nursing or a related field.
- Licensed to Practice as a Registered Nurse in the Commonwealth of Virginia.
- American Heart Association (AHA) Health Care Provider BLS certification.
- Relevant specialty board certification strongly preferred.
Physical Demands:
- Standing for prolonged periods.
- Frequent travel.
- Bending and stooping.
Work Environment:
- May be exposed to chemicals, blood/body fluids, and infectious disease.
