About the Role

Aveanna Healthcare is seeking a skilled and compassionate Private Duty Nurse LVN to join our team of dedicated caregivers. As a Private Duty Nurse, you will play a vital role in providing one-on-one care to our patients in the comfort of their own homes.

Key Responsibilities
  • Implementing physicians' prescribed plans of care to ensure optimal patient outcomes
  • Conducting patient assessments and coordinating care with our multidisciplinary team
  • Administering prescribed medication, treatments, and therapies as ordered by physicians
  • Documenting care provided in our electronic system of record, ensuring compliance with regulatory requirements
  • Providing health education and training to patients and their families, promoting healthy habits and disease prevention
  • Collaborating with our Case Manager to ensure seamless care transitions and address patient concerns
Requirements
  • Active, unencumbered license (RN/LPN/LVN) in the state of practice
  • Current CPR certification from the American Red Cross or American Heart Association
  • TB skin test (current within the last 12 months)
  • At least six months of prior hands-on nursing experience, with a strong focus on home care
  • Reliable transportation to ensure timely patient visits
